Superior Labrum Anterior and Posterior (SLAP) Tear in Sports

A common sports injury, a Superior Labrum Anterior and Posterior tear, or lesion, is an injury to the labrum. The labrum is the ring of cartilage that surrounds the socket of the shoulder joint, also acting as an attachment point for many ligaments in the shoulder. The labrum’s role in the shoulder is to stabilize the shoulder joint and its movements, as well as assist in deepening the socket of the joint. With athletes’ shoulders being an integral part of throwing and lifting, a SLAP tear can easily occur over time.

Causes of a SLAP Tear

Either acute trauma or repetitive shoulder movements usually cause SLAP tears. Athletes participating in sports such as baseball, softball or football are more susceptible to SLAP tears as they all require overhand throwing. Repetitive overhand throwing usually causes the labrum to tear over time. Acute trauma from falling on an outstretched arm, or a sudden movement in a game with the arm outstretched or above the shoulder can also cause a SLAP tear.

Symptoms of a SLAP Tear

Athletes, who have experienced a SLAP tear, commonly report feeling popping, clicking and catching sensations in their shoulder following the injury. Other symptoms include aching, pain lifting the arm above the shoulder or throwing a ball and weakness or instability when holding or lifting items of various weights. However, some symptoms of SLAP tear can be hard to determine without MRI testing or arthroscopic surgery.

Treatment of a SLAP Tear

Each SLAP tear is different. The NJOI physicians will decide a treatment plan based on the location of the tear, the severity and the long-term goals of the patient. If a patient’s injury is less severe, a non-surgical approach can be taken. This type of treatment includes medicine, such as ibuprofen and naproxen, to help reduce pain and swelling in the shoulder due to movement. Physical therapy may also be recommended to patients to improve their flexibility and range-of-motion in the shoulder joint. The physical therapy exercises will take the stress off the labrum and prevent the risk of a future shoulder injury. If a patient has a full SLAP tear, arthroscopic surgery is required to repair the injury. Through arthroscopic surgery, our experienced surgeons are able to remove and reattach torn sections of the labrum.
